Women with Persistent Acne Had a New Option That Worked as Well as Accutane with Fewer Side Effects

JAMA Dermatology · March 12, 2026

The Bottom Line

Low-dose spironolactone achieved comparable acne clearance rates to low-dose isotretinoin in women with persistent adult acne, with fewer reported side effects over 24 weeks.

What Happened

A double-blind randomized controlled trial enrolled 316 women aged 25-40 with moderate-to-severe persistent acne. Half received spironolactone and half received low-dose isotretinoin for 24 weeks. Both groups showed similar reductions in inflammatory lesions, but the spironolactone group reported significantly fewer side effects (18% vs. 41%).
